Stop by for a FREE intake & Case Assessment:

• Discuss your current needs related to getting into medical care
• Find insurance plans as needed
• Address any concerns you may have about getting medical care
• Complete initial lab work
• Schedule your first medical appointment

Please Bring the following with you:

• 1 month of income for everyone who is working in the household, or we will provide you with a "Verification of No Income" form if you do not have an income
• Picture ID
• Birth Certificate or US Passport
• Social Security card
